To apply for medical support from The Gus and Buddy Fund, please complete the application provided on this page. We kindly ask that you fill out the entire form before submitting.
Though we strive to provide support to as many cases as possible, not all cases will be funded. Funding depends on a variety of factors including fund availability, medical diagnosis, and cost of treatment. While The Gus and Buddy Fund will provide a certain amount of money to assist in the care of your pet, a financial commitment from the client/pet parent is also required.
If application information is incomplete, your pet has not seen a veterinarian, or you cannot provide a medical diagnosis, then the application will not be considered.
If financial resources from the fund are allocated to the care of your pet, payments will be made directly to the veterinary hospital.
